A place of business used in the care and preparation or transportation of human remains, or any place where one of more persons, either as sole owner, in partnership, or through corporate status, are engaged or represent themselves to be engaged in the funeral business.
Funeral Establishments must be licensed to practice funeral directing and/or embalming in the State of Tennessee.
Requirements

3) Corporation / LLC:
a) List of officers, titles, addresses and registered agent
b) Copy of organization from Secretary of State
4) Letter from Zoning Authority stating that the property is specifically zoned for a funeral esablishment or crematory
5) Parking requirements
6) City Sewer or Septic Tank (If septic tank – copy of letter of approval from County Health Department)
7) If no preparation room, signed letter from Establishment providing preparation of deceased human remains
8) Employee List
a) Names and addresses
b) License numbers for Funeral Directors and/or Embalmers
c) Status of employment (full-time or part-time)
9) General Price List
10) Casket Price List
11) Outer Burial Container Price List
12) Statement of Funeral Good and Services Selected Contract

Additional
Name Change:
1. Updated price lists must be available at time of inspection.
2. Licensed Manager or owner to appear in front of the board.
3. $260.00 fee must accompany application ($60.00 database change; $200.00 reinspection fee).
Location Change:
1. Updated price lists must be available at time of inspection.
2. If establishment is on septic system, attach letter of approval of septic system from the county health department.
3. Letter from zoning authority to verify location is properly zoned for funeral establishment.
4. $575.00 fee must accompany application.
Grace Period:
The late fee is $200 and the grace period is 90 days.
After grace period, the licensee has to submit a reinstatement request by email. They will then have to go through whole process of applying for an initial license.
Change of Address:
For a Funeral Director or Embalmer, a change of address is reported through your core.tn.gov account. After you log in or create an account, you will choose the “My Licenses or Permits” tab, select your credential, then select the Change Address option.
For funeral establishments, you need to complete a Change of Location application within your core account at core.tn.gov and email the board if you would like to add a PO Box for your mail. The mailing address for funeral establishments has to be at your physical location unless you add a PO Box.
